Research Application, Herbert Hoover Presidential Library

Below is the standard application form used by all of the presidential libraries administered by the National Archives and Records Administration. The second page of this form includes research room rules enforced by all facilities administered by NARA.

Please print this form if you plan to conduct research in our Research Room. When completed, please mail or fax the form to the Hoover Library. (Mailing address: P.O. Box 488, West Branch, Iowa 52358; Fax: 319-643-6045). This process will facilitate your research in West Branch and allow our archivists to prepare for your visit. Thank you for your assistance.
